F.A.Q.’s

1. Will my insurance cover this? 
Our specialists in the billing department will contact your Insurance and determine how your insurance poliy will cover your new orthotic or prosthetic.  The office assistant will notify you of your portion due prior to the delivery of your item. It is important to remember that nothing is ever a guarantee of payment therefore you may be responsible for a portion of the services received from Evergreen Prosthetics & Orthotics.

Each insurance policy is different.  We will assist you in receiving the maximum benefits possible and, at your request, will assist in submitting your claims. The final responsibility for payment remains with you.  We encourage you to ask to speak with one of our specialists if you have additional questions.

2. How long will it take to get my device?
Each patient receives individualized treatment. From the evaluation, we determine what your treatment plan will require and what type of prostheses/orthoses is best suited for your needs. The fabrication of this device may require impressions, castings, measurements. moldings,  and models. On average, most orthotic devices will take two-three weeks to be completed. Prosthetics, however, can take longer. Due to the specialization and individualization of our practice, it is best to consult your practitioner at your initial visit to work with him/her on the amount of time needed to make your device.

3. Do I need an appointment?
Yes, please! We see all our patients on an appointment basis and request that you call in advance so we can reserve a time for you. In the case of emergencies, please contact the nearest office and notify them that you need to be seen as soon as possible. Our offices will do their best to schedule you as quickly as possible.

If you cannot keep an appointment, please notify us immediately. We ask that you give us 24 hours notice.

4. How do I care for my device?
When you receive your device, the practitioner will go over the information with you. In addition, you will have the opportunity to discuss the care of your device or any other questions throughout the entire process.

5. What do I need to bring to my appointment?
For your initial appointment, we ask that you arrive 10-15 minutes prior to your actual time in order to allow for the necessary paper work to be completed. At your appointment you will need to have a photo id, your insurance card and your prescription/referral. If you have Medicare or Medicaid, please be sure to have a current copy of your card with you.

Please note that at the initial visit you will be asked to complete a patient information sheet. The patient information sheet requests general information such as phone number, address, social security number, date of birth and so forth.

6. What types of insurance do you accept?
Evergreen Prosthetics & Orthotics accepts almost every HMO’s, PPO’s and other commercial insurance company providing coverage in Oregon and Washington. In addition, we are a provider for Medicare, and Medicaid. If you do not see your insurance policy on our list of providers please contact one of our offices for additional assistance.

7. Do you accept Medicaid? What is the Medicaid process?
Yes, we accept Medicaid. You must first call to schedule an appointment. Please let the patient care coordinator know that you have Medicaid when calling to schedule your appointment. On your initial visit, please be sure to have your current Medicaid card with you.

Please note prior approval may be needed for some devices and may therefore affect the timeliness of your care. At anytime, please feel free to contact any of our offices with questions or concerns regarding Medicaid, its processes and procedures.

8. Do you accept Medicare? What is the Medicare Process?
We accept Medicare, please contact any one of our offices for additional assistance.

9. What is the policy for adjustments?
Every Patient has a unique situation, and therefore adjustment schedules will be decided between you and your practioner.

10. Who do I contact if I have a question about a bill?

11. Where is Evergreen Prosthetic & Orthotics located?

12. Can I go to any Evergreen location?
